Family History Questionnaires

Following your referral to the Genetic Clinic, you may have been asked to complete a family history questionnaire. Electronic versions of the questionnaire are available.

If you have been referred due to a personal and/or family history of cancer: please complete and return this questionnaire.

For any other referral unrelated to cancer: please complete and return this questionnaire.

Completed questionnaires can be returned to us via email to gram.clinicalgenetics@nhs.scot.

Please only send completed forms as a pdf or word document as other formats will not be accepted.

Please be aware that we will not accept the questionnaire if you have not been referred to Genetics and have not been asked to complete one.
