To register with Whinhill Medical Practice please complete the following two forms and send them into the practice:

Patients who are registering with the practice from non-EU countries need to check the government web site to ensure they have the relevant paperwork required before they can register for free NHS care.

 

Patients are registered with the practice and not with a specific doctor. However you can express a preference to see any doctor at anytime. The doctors operate a rota system therefore appointment availability with a specific doctor will be dependent on this. We will do our best to accommodate your preferences. You will be registered with the practice within 3 days once all checks have been completed.

 

The practice would appreciate any change of circumstance patients may have, for example change of address or telephone number; getting married and change of name (documentation to prove this will be required).  Children who reach the age of 14 years no longer need parental consent to book or cancel their own appointments.
