What is my MyPreOp?
MyPreOp is an online pre-operative assessment questionnaire which can be completed via your smartphone before your procedure. Ultramed is the company that provides this service to patients on behalf of NHS Grampian.
MyPreOp will ask you questions about you, your health, and your operation. MyPreOp generally takes between 30-60 minutes to complete but, depending on your individual circumstances and medical history, may take longer. Please be as honest as possible answering these questions so we can ensure you are appropriately assessed for anaesthesia.
You do not need to complete it all at one time because your progress is saved as you enter it. The NHS Grampian clinical team may contact you after this initial assessment if they require more information. They will also organise any relevant appointments and investigations in preparation for surgery if required.
When should I complete MyPreOp?
Please complete the MyPreOp questionnaire as soon as possible, aim to complete it within 7-10 days of receiving the invitation. This will avoid any delays with your pre-assessment or your planned surgery.
How do I access MyPreOp?
You will receive an invite by text message or email which will have a link to your questionnaire. You will be asked to enter your date of birth to be able to complete the questionnaire online. If you are unable to complete MyPreOp by yourself, a relative, carer or friend can help you complete it. If you are unable to gain access to the internet or do not have a smartphone, the pre-operative assessment team will arrange for a telephone or in person clinic appointment, where staff will help complete the MyPreOp questionnaire with you.
What happens if I do not complete my questionnaire in one sitting and need to return to it later?
To pick up where you left off in completing the questionnaire use the link sent to you. You will be prompted to enter the last 12 digits of this link (referred to as a unique access code). You will then be prompted to enter your date of birth and be able to continue to complete the questionnaire.

What are the benefits of MyPreOp?
- It may avoid you having to visit the hospital for a face to face pre-operative assessment, saving you time, travel, parking, inconvenience and time off work.
- If you do need to attend our pre-operative assessment clinic, that appointment will likely be shorter. An appointment will be organised with the appropriate professional based upon your individual needs. This could be a telephone call or a face-to-face appointment depending on your health needs.
- You can complete MyPreOp at a time that is convenient to you, and you have the time you need to carefully consider your answers.
- Using MyPreOp reduces the use of paper to support our carbon neutral goals.
Is my information secure?
Ultramed takes great care to make sure healthcare information is kept secure. Data is always encrypted when sent to them, as well as when it is stored. Only those NHS Grampian staff involved in your care are able to see the information you provide.
What if I don’t have a smartphone or cannot do this myself?
If you are not confident at using digital technology, you can ask a family member or carer to help you. If you have no one able to help, or do not have a mobile phone number or email address, you will still be pre-assessed. The pre-operative assessment team will contact you if you are unable to complete your assessment questionnaire and you will be offered an in clinic or telephone appointment.
Who do I contact if I have questions about the pre-assessment process?
NHS Grampian has three pre-operative assessment clinics. You will have been referred to one of these clinics depending on where you live and what type of surgery you are having.

Who do I contact if I have any questions about my surgical procedure?
Please phone the number on the letter you would have received at the time you spoke to a Consultant about having surgery. You can call the NHS Grampian switchboard on 0345 456 6000 and ask to be put through to the secretary for the Consultant you saw at your out patient appointment.
What if i have Interpretation or Translation needs?
NHS Grampian provides interpretation and translation services for people who:
• cannot speak or find it difficult to speak in English
• who are Deaf and use British Sign Language
• are Deafblind
If you will require any of these services these can be arranged by the pre-operative assessment team you have been referred to.
