Consultant Appraisal

Please read these instructions regarding how to fill out the appraisal forms.

Below you will find three versions of the Consultant Appraisal paperwork.  There is paper work for the Consultant, for Joint Academic and Clinical Appraisal and for Consultant Career Grade / SAS Doctor.

Each form is designed to be filled out, on screen.  You can use the 'tab' button or the down arrow on your keyboard to move between questions. Remember to save your document in a place you will remember and to save your work regularly so you do not loose anything.
