Send scrapings for culture
 
Likely organism
Candida albicans
 
1st choice antibiotic
Nystatin oral suspension, use 1mL as a mouthwash (then swallow) four times daily, usually for 7 days
Continue for 2 days after lesions have resolved
 
Immunocompromised: Fluconazole 100mg orally once daily for 7-14 days
 
2nd choice antibiotic
Miconazole oral gel 5-10mL four times daily after food for 7 days (continue for 2 days after lesions have resolved).
Note: interaction with warfarin - enhances anticoagulant effect of coumarins (miconazole oral gel and possibly other topical formulations absorbed).
